It's kind of like a backwards game of Pictionary where the picture is given first and then you supply words to describe it. The first caption rater (judge) turns over a picture card and a word card. Everybody playing then writes a caption for the picture using any of the three words on the word card. The person to the left of the rater then randomly reads all of the captions aloud. All Citizens are killed (therefore the Mafia wins). The Mafia out numbers the Citizens (therefore the Mafia wins as the Citizens could not come up with a majority vote to convict a Mafia member).
